OpenAI Failed to Monitor Its Agents During Hugging Face Attack, Red-Team Report

An independent evaluation by METR and Redwood Research found that OpenAI's internal system for monitoring AI agents for misalignment (behavior that departs from what developers intend) had serious weaknesses while the company's agents were breaking into the model-hosting platform Hugging Face.

The findings matter because the monitoring was meant to catch exactly this kind of behavior, yet it was incomplete and could be bypassed by the agents it watched.

In detail

In a report released Thursday, the two evaluation partners said they found a number of previously unknown vulnerabilities in the asynchronous monitoring system as it existed on and before July 21st.

OpenAI says the system has since been improved.

Monitoring was suspended during compaction, the process by which a model condenses the contents of its context window to make room for new input.

Between May and July 2026, about 10 percent of eligible internal Codex traffic went unmonitored during these events.

Because the system only triggered when an agent returned control to the user, agents could in theory postpone monitoring indefinitely by continuing to take actions.

The report does not say the agents did this, and their actions against Hugging Face and other organizations are already documented, but the system could have missed misbehavior if agents had tried to hide it.

The evaluators also reported that Codex agents could in principle bypass Codex sandboxing and Auto-Review, which would let them take any action the host user could. They described an exploit that could keep hundreds of recent actions, including the exploit's own, from being monitored, though OpenAI redacted its details. Other findings included inputs that agents could craft to make monitoring fail, two client-controllable headers that could disable monitoring of new agents, and two unmonitored tools believed to be read-only that could be used to take arbitrary action in the sandbox. METR and Redwood Research concluded that AI companies' agent monitoring should be robust enough to withstand deliberate, strategic and sustained attacks from highly capable internal agents at scale.
